ABSTRACT:
Background: Road traffic injuries remain a major public health concern worldwide, with drivers often acting as the first responders at crash scenes. Adequate first-aid knowledge and a positive attitude toward providing immediate care can significantly reduce morbidity and mortality. Visual instructional methods, such as videos, infographics, and simulation-based training, have emerged as effective educational tools for enhancing learning outcomes in first-aid education. Aim: To evaluate the systematic review on effectiveness of visual instructional methods in improving first aid knowledge and attitude among drivers. Methodology: A systematic review was conducted following PRISMA guidelines. Relevant studies were retrieved from databases including PubMed, Scopus, Web of Science, CINAHL, and Google Scholar (2000–2025). Studies involving drivers exposed to visual instructional interventions such as videos, animations, simulations, and multimedia-based learning were included. Eligible studies assessed outcomes related to first aid knowledge and attitudes. Data were extracted using a standardized tool and quality appraisal was performed using appropriate critical appraisal checklists. Due to heterogeneity, findings were synthesized narratively. Results: A total of studies met the inclusion criteria, comprising randomized controlled trials, quasi-experimental studies, and pre-post intervention designs. Overall, visual instructional methods led to a substantial improvement in first aid knowledge, with reported increases ranging from 35% to 70% in post-intervention scores compared to baseline. Approximately 65%–85% of participants demonstrated adequate knowledge following training, compared to 20%–40% prior to intervention. Attitudinal outcomes also showed significant enhancement, with 60%–80% of drivers expressing increased confidence and willingness to administer first aid after exposure to visual learning tools. Multimedia interventions combining video demonstrations and interactive elements were associated with the highest effectiveness, showing up to 75% improvement in knowledge retention and a 70% increase in positive attitudes. However, variations in study quality, sample size, and intervention duration were observed across included studies.
